About Us
Building Bridges To Better Care
Heart Bridge Care is a San Francisco Bay Area not for profit that was born out of a shared realization: while the modern healthcare system excels at treating the body, it often leaves patients and families to navigate the emotional and logistical realities of severe illness and the dying process entirely on their own.
Today, Heart Bridge Care serves our community through two integrated missions: providing direct, compassionate end-of-life navigation for our community, and offering high-level clinical and holistic mentorship for the next generation of end-of-life doulas (EOLD).
